
DOT Medical Card - Registration with Secretary of State’s (Michigan) office.
“Beginning January 30th, 2012 the Secretary of State will begin collecting the self-certification of driving type and also the Medical card, if applicable. The four driving types (very briefly) are:”
{Non-excepted interstate, excepted interstate, non-excepted intrastate, excepted intrastate}
“Michigan will only collect the DOT medical card if “Non-Excepted Interstate” is chosen.”
“If you are not sure which type of CMV operation you will perform, please get confirmation from your employer or prospective employer.”
Drug Screens Plus note: The definition of “Commerce” includes…”which…affects any trade, traffic, and transportation described in…” You may be involved in “interstate” commerce even when you think you are not. Don’t assume that you are in “intrastate” commerce simply because you do not leave the state.
Any CDL driver that applies for an Original, Correction or Renewal CDL on or after 01/30/2012 must certify to their driving type and present a valid DOT medical card if “Non-Excepted Interstate” is chosen. Drivers will be denied until they present these documents. All drivers must self-certify to their driving type and provide the Secretary of State their Medical card, if applicable, by January 30, 2014 or their CDL privileges will be automatically downgraded to non-commercial. Drivers will receive notification of the impending downgrade 10 days prior.
You may complete your self-certification (form for Michigan not available at date of this notice) of driving type and submission of DOT card (if applicable) the following ways:
WEB: Self-certification form will be available for on-line submission and your
medical card may be scanned (Website information will be announced in
the near future).
FAX: Michigan CDL Help Desk 517-636-4359 (must include cover sheet with
phone number).
MAIL: Mail your completed self-certification form and DOT card to:
